About the Role:
In anticipation of future government awards, GiaCare Inc. is now accepting applications for Registered Nurse Case Managers to support Fort Campbell near Oak Grove, Kentucky.
Education:
- Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), the Commission on Collegiate Nursing Education (CCNE) or individuals attending a master’s level bridge program in nursing who have completed coursework equivalent to a bachelor’s level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an Page 24 of 53 approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation.
Experience:
- One year of experience in nursing as identified in after graduation.
Certifications:
- Basic Life Support (BLS)
- Possess one of the following certifications:
- Commission for Case Manager Certification Certified Case Manager (CCM)
- Certification of Disability Management Specialists Commission: Certified Disability Management Specialist (CDMS)
- Association of Rehabilitation Nurses (Certified Rehabilitation Registered Nurse (CRRN)).
- American Board for Occupational Health Nurses Certified Occupational Health Nurse (COHN)
- Certified Occupational Health Nurse-Specialist (COHN- S)
- National Board for Certification in Continuity of Care (Advanced Certification in Continuity of Care (ACCC))
- Commission on Rehabilitation Counselor Certification (Certified Rehabilitation Counselor (CRC))
- American Nurses Credentialing Center Nurse Case Manager (RN-NCM)
- National Academy of Certified Care Managers (Care Manager Certified (CMC)).
- In case of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an
appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Credit for foreign nursing education higher that associated degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware.
Duties:
- The Registered Nurse (Case Manager) shall provide competent, evidence-based care to assigned patients.
- Provide patient management, care coordination, and discharge/disposition planning for outpatient settings.
- Assist family members and caregivers with receiving the most appropriate options and services to meet their complex health care needs. This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems.
- Coordinate care with multiple providers across all levels and sites of care.
- Address psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in interdisciplinary patient care management practices.
- Evaluate care and outcomes to ensure timely and appropriate provisions of services.
- Coordinate and implements discharge plans by arranging for transition to the next level of care, in compliance with the multidisciplinary treatment plan.
- Be responsible for documenting outcomes at the program or service level.
- Execute position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex patient care.
- Evaluate outcomes of evidence-based decisions and practice changes for individuals, groups, and populations.
- Program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a facility.
- Comply with regulatory, MTF, and Joint Commission quality inspection/ peer review/ utilization management requirements, and applicable system applications.
- Complete training to obtain knowledge and maintain competency in the following systems: MHS Genesis, Dragon dictation software, and Microsoft Office programs.
- Contractor employees shall prepare all documentation to meet or exceed established MTF standards, to include, but not limited to: timeliness, accuracy, content, and signature. Contractor employees shall only use DoD approved abbreviations for documentation in the patient health records. (DoDI 6025.13 and DoDM 6055.05)
- Document and close notes in MHS Genesis within three (3) business days of patient encounters.
- Respond to beneficiary messages submitted and received in the MHS Genesis secure portal systems within one (1) business day.
