Job Details
- Location: Clark County, Washington
- Salary: $120,000 - $150,000 annually
- Employment Type: Permanent
- Hours Per Week: 40
- Shift Duration: 8 Hour Days
Job Requirements
- Must have a valid unencumbered Nurse Practitioner License for the state you will be working in
- Previous in-home risk assessment experience preferred
- 3 years of patient care experience preferred (primary care/adult/geriatric, EMR)
- Access to reliable transportation for travel within the assigned area
- Strong ability to work within our EMR system
- Ability to work independently
- Bilingual skills are a plus
Responsibilities
- Perform annual in-home wellness visits and risk adjustment health assessments
- Provide individualized quality care to the elderly population in their homes
- Deliver patient health education on topics such as pain management and medication
- Build trust with members through exceptional communication and empathy
- Assist in closing quality care gaps (e.g., screenings and labs)
- Collaborate to enhance care delivery
- Travel throughout the assigned state, within a 55-mile radius
- Other duties as assigned
