**Job Title:** LPN / LVN Long Term Care
We are seeking a skilled LPN/LVN to join our team in providing high-quality care to patients in a long-term care setting.
**Responsibilities:**
- Provide hands-on nursing care to patients, including administering medications, performing treatments, and monitoring vital signs.
- Conduct patient assessments, develop care plans, and implement interventions to achieve desired outcomes.
- Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions between shifts.
- Document patient information, including medical histories, progress notes, and discharge summaries.
**Requirements:**
- Current LPN/LVN licensure in Pennsylvania.
- Experience working in long-term care or similar settings.
- Ability to work night shifts, 8 hours per week.
