Home Health Case Manager

Job Description

We are a Home Health Agency in Thousand Oaks, CA that is looking for a self motivated, compassionate RN or LVN Home Health Office Manager! (Non-licensed clinicians/medical staff are encouraged to apply so long as they have previous home health experience)

The job requires the ability to utilize a PC, Printer, multi-line telephone, and other related office equipment. Staff will work from office. We do not offer work remotely.

Responsibilities

Schedule is Monday-Friday 8:30am-5pm.

Coordinates the patient’s and families care and services including those of the team, social worker, home health aide and physician

Responds necessary information to the Director of Nursing Monitors the re-certification / re-authorization process

Ensures and documents appropriate use of the medications, durable medical equipment and supplies

Educates patients, families and other caregivers Appropriately utilizes the skills and resources of contract personnel such as PT, OT, Podiatrist and Pharmacy

Provides appropriate care plan and integrates care plans as needed

Participates in routine chart reviews Attends regularly scheduled in-services and educational conferences.

Qualifications

• Must be LVN/RN. (Non-licensed clinicians/medical staff are encouraged to apply so long as they have previous home health experience)
• Must have a minimum of 1-year Hospice or Home Health Care experience, (ability to use Axxess Home Health Software)
• Must possess typing and other clerical skills.
• Fluent in English speaking and writing.

Salary: Starting at $41,600 per year PLUS Mileage and Bonuses. (*If unlicensed nurse, starting pay is subject to change.)

Experience: Home health/Hospice care: 1 year (Required)

Medical terminology: 1 year (Required) Computer skills: 1 year
