Responsible for engaging with community clients to assess their individual needs and connect them with supportive services and resources. This position plays a key role in fostering self-sufficiency by offering guidance, resource coordination, and hands-on support. Collaborates closely with program staff to support broader agency goals and service delivery.

Ability to communicate with non-English speakers. Key Duties and Responsibilities:

  • Develop and Execute Outreach StrategiesoDesign and implement targeted outreach plans to engage underserved populations and promote organizational programs.oIdentify and prioritize high-need communities, ensuring efforts are aligned with organizational goals and community trends.oProactively connect with the growing non-English-speaking population using multilingual communication and cross-cultural understanding.
  • Foster Community Partnerships and EngagementoBuild and sustain relationships with local organizations, community leaders, block clubs, and stakeholders to expand service reach and foster collaboration.oRepresent the organization at public meetings, coalition groups, and forums to advocate for community needs and share program impact.
  • Coordinate and Support Community EventsoPlan, organize, and participate in events, workshops, and resource fairs to increase visibility and accessibility of services.
  • Deliver Direct Client Support and Case ManagementoProvide individualized case management, needs assessments, and referrals to address housing, health, education, and financial needs.oConduct ongoing follow-ups to monitor progress, resolve barriers, and adjust service plans accordingly.
  • Promote Access for Non-English-Speaking ClientsoEngage linguistically isolated clients through ESL program enrollment and personalized language-access support.oOffer bilingual assistance and culturally appropriate services for clients speaking other languages.oSupport the growing non-English-speaking community by leveraging multilingual skills and cultural understanding to build trust and bridge service gaps.
  • Collaborate Across Programs for Holistic Service DeliveryoWork in partnership with internal teams, including preventive services and meal programs, to ensure coordinated care and consistent client engagement.
  • Provide Support Through Food Assistance ProgramsoEngage with clients accessing the take-out meal program, offering crisis assistance, resource referrals, and broader case management as needed.oUse meal distribution as a touchpoint for connecting clients to long-term support services.
  • Conduct Community Needs AssessmentsoGather feedback from clients and community partners to identify emerging issues and gaps in service.oUse findings to inform outreach priorities, program enhancements, and strategic planning.
  • Distribute Resources and InformationoDisseminate educational materials, program information, and resource guides in community spaces and across digital platforms, including social media and email newsletters.
  • Monitor Program Data and Report OutcomesoCollect and analyze data on service utilization, outreach efforts, and client outcomes to inform decisions and report progress to funders and stakeholders.oMaintain accurate records and contribute to evaluation and continuous improvement efforts.
  • Advance Equity, Inclusion, and Cultural CompetencyoEnsure all outreach and services are inclusive, culturally responsive, and sensitive to the needs of diverse communities.oAdvocate for language access, equity in service delivery, and client empowerment at every level.
  • Provide flexible support to program staff and assist with agency-wide initiatives as needed.
  • Perform other duties as assigned to meet program and organizational goals.Requirements: Bachelor’s degree or foreign academic equivalent in any field and 2 years experience in Health and Human Services. Requires fluency in Bengali and Hindi.
