We are seeking a reliable and detail-oriented Bilingual Field Coordinator to oversee client services, ensure quality assurance, and support both clients and caregivers.
Key Responsibilities
- Complete and review weekly and monthly case notes in accordance with agency and regulatory requirements
- Perform home visits to monitor care quality, assess client needs, and ensure caregiver compliance
- Identify, document, and report any client or caregiver issues, incidents, or concerns to management promptly
- Monitor caregiver attendance, performance, and case continuity
- Assist with care plan updates and service adjustments as needed
- Maintain confidentiality and compliance with all agency, state, and Medicaid/DSS requirements
Qualifications
- 1–2 years of experience in home care coordination, case management, or healthcare services (preferred)
- Strong organizational and documentation skills
- Excellent communication and interpersonal abilities
- Ability and willingness to conduct home visits and field work
- Attention to detail and ability to manage multiple cases effectively
- Familiarity with EVV systems, case notes, and QA processes (preferred)
- Commitment to quality care and client advocacy
