We are seeking a reliable and detail-oriented Bilingual Field Coordinator to oversee client services, ensure quality assurance, and support both clients and caregivers.

Key Responsibilities

• Complete and review weekly and monthly case notes in accordance with agency and regulatory requirements
• Perform home visits to monitor care quality, assess client needs, and ensure caregiver compliance
• Identify, document, and report any client or caregiver issues, incidents, or concerns to management promptly
• Monitor caregiver attendance, performance, and case continuity
• Assist with care plan updates and service adjustments as needed
• Maintain confidentiality and compliance with all agency, state, and Medicaid/DSS requirements

Qualifications

• 1–2 years of experience in home care coordination, case management, or healthcare services (preferred)
• Strong organizational and documentation skills
• Excellent communication and interpersonal abilities
• Ability and willingness to conduct home visits and field work
• Attention to detail and ability to manage multiple cases effectively
• Familiarity with EVV systems, case notes, and QA processes (preferred)
• Commitment to quality care and client advocacy
