Job Description:
Georgia B. Divine Care, a licensed non-medical personal service agency based in Hopkinsville, Kentucky, is seeking a compassionate and experienced Registered Nurse (RN) to join our team on a PRN (as-needed), per-visit basis.
As a newly credentialed provider with the Veterans Affairs (VA) Community Care Network, we are committed to delivering high-quality, person-centered care to our nation’s veterans in the comfort of their homes.
The RN will play a critical role in ensuring clinical oversight, regulatory compliance, and the development and monitoring of individualized care plans for veteran clients.
Key Responsibilities:
- Conduct initial in-home assessments for veteran clients to determine care needs and service eligibility
- Complete ongoing reassessments in accordance with VA requirements and agency policies
- Develop and review individualized care plans in collaboration with caregivers and interdisciplinary teams
- Provide clinical supervision and oversight of non-medical caregiving staff
- Monitor client condition and report any changes in health status, safety concerns, or incidents in a timely manner
- Ensure compliance with VA Community Care Network guidelines, state regulations, and agency policies
- Maintain accurate, timely, and detailed documentation of all visits and client interactions
- Participate in care coordination with VA representatives, case managers, and other healthcare professionals as needed
- Support quality assurance and continuous improvement initiatives within the agency
Compensation:
This is a PRN, per-visit position.
Compensation ranges from:
- $100 – $150 per visit (Initial Assessments)
- $75 – $100 per visit (Reassessments & Supervisory Visits)
Visits are scheduled based on client needs. This position is ideal for an RN seeking flexible, supplemental income.
Qualifications:
- Active and unrestricted Registered Nurse (RN) license in the state of Kentucky
- Minimum of 1 year of nursing experience (home health or community-based care preferred)
- Strong knowledge of patient assessment, care planning, and documentation standards
- Ability to work independently and manage a flexible schedule
- Excellent communication and interpersonal skills
- Valid driver’s license and reliable transportation
- Ability to pass background check and meet VA compliance requirements
Preferred Skills & Experience:
- Experience working with veterans or VA programs
- Knowledge of home and community-based services (HCBS) or waiver programs
- Familiarity with electronic documentation systems (EVV/Therap preferred)
- Strong clinical judgment and attention to detail
- Ability to identify and respond to risk, safety concerns, and changes in condition
- Experience in staff supervision or training
Work Environment:
- In-home client visits within the service area (Christian County and surrounding areas)
- Flexible scheduling based on client assignments
- PRN/as-needed workload
