Community Relations & Medicaid Onboarding Coordinator (Part-Time)

We are a growing non-medical home care agency looking for a motivated and organized individual to help grow our community presence and support the onboarding of new Medicaid clients.

Hours: 5 hours per week to start, with the opportunity for additional hours as the agency grows.

Responsibilities

• Build relationships with hospitals, rehabilitation facilities, physician offices, senior centers, and community organizations to generate referrals.
• In-person visit these companies to build relationships with key providers.
• Represent the agency at networking and community events.
• Assist with Medicaid client intake and onboarding.
• Coordinate with case managers and referral sources.
• Ensure all required onboarding documentation is completed accurately and on time.

Qualifications

• Experience in home care, healthcare, Medicaid, or community outreach preferred.
• Strong communication and organizational skills.
• Self-motivated, dependable, and comfortable working independently.
• Reliable transportation for local community visits.
