The Community Living Supports (CLS) Specialist provides individualized support to children and adults receiving North Carolina Medicaid 1915(i) Community Living and Supports services. The CLS Specialist assists individuals in developing independent living skills, increasing community integration, improving social skills, and achieving goals identified in the Person-Centered Plan (PCP).

Essential Duties and Responsibilities

  • Provide one-on-one Community Living Supports services according to the individual’s Person-Centered Plan.
  • Assist individuals with activities of daily living (ADLs) and independent living skills as authorized.
  • Teach communication, coping, socialization, and problem-solving skills.
  • Support community integration through recreational, educational, vocational, and social activities.
  • Assist with transportation to approved community activities and appointments when authorized.
  • Encourage independence while maintaining the individual’s dignity and choice.
  • Monitor health, safety, and well-being during service delivery.
  • Follow behavioral support plans and crisis intervention procedures when applicable.
  • Report incidents, injuries, abuse, neglect, exploitation, or safety concerns immediately.
  • Maintain confidentiality in accordance with HIPAA regulations.
  • Complete service documentation and progress notes accurately and within required timeframes.
  • Utilize electronic visit verification (EVV) and other required documentation systems.
  • Attend required staff meetings and training.

Qualifications

  • High school diploma or GED required.
  • Associate’s or Bachelor’s degree preferred.
  • Experience working with individuals with intellectual/developmental disabilities, mental health conditions, traumatic brain injury, or substance use disorders preferred.
  • Valid driver’s license, reliable transportation, and automobile insurance.
  • Ability to pass criminal background and registry checks.
  • Current CPR/First Aid certification (or willingness to obtain).

Pay: $20.00 per hour
