Gentiva is seeking a highly skilled Nurse Practitioner to join our team in West Branch, Michigan. As a Clinical Hospice Care Specialist, you will play a vital role in providing compassionate care to patients and their families.
The scope of this position includes performing Face-to-Face (F2F) encounters, gathering clinical information to support hospice diagnosis and prognosis, and communicating findings to the hospice physician responsible for certifying or recertifying patient eligibility.
To be successful in this role, you must possess a Master's or higher degree in Nursing, completion of a graduate-level nurse practitioner education program accredited by a national accrediting body, and current state licenses as a registered nurse and advanced practice nurse. Additionally, you must have a collaborative agreement in place and national certification as a Nurse Practitioner.
