Role Description
This is a full-time, on-site position located in Pittsburgh, PA. As a Registered Nurse Hospice RN Case Manager, you will provide compassionate and high-quality nursing care to hospice patients. Key responsibilities include managing individualized care plans, administering medications, offering palliative support, coordinating with other healthcare providers, and educating families on patient care. This role is critical in ensuring patients' comfort and dignity during end-of-life care and supporting their loved ones throughout the process.
Qualifications
- Strong expertise in Nursing and Direct Patient Care
- Knowledge and experience in Palliative Care and Medicine
- Proficiency in Home Care delivery practices
- Excellent communication and interpersonal skills to support patients and their families
- Active and valid RN license in the state of Pennsylvania
- Experience in hospice or end-of-life care is highly desirable
- Ability to work collaboratively within an interdisciplinary team
- Demonstrated organizational skills and attention to detail in care coordination
