Job Summary:
The Medical Records Clerk plays a vital role in supporting and preparing medical records for residents and staff. This position works closely with residents, families, and the interdisciplinary care team to ensure high-quality care and smooth transitions throughout a resident’s stay.

Key Responsibilities:
- Works closely with Admissions and Care Transitions Manager to drive admissions and ensure a smooth admissions process for patients.
- Complete necessary forms for admission of a resident
- Complete necessary forms and make arrangements for resident's discharges
- Orient new residents to the facility
- Must be able to read and write medical terms
- Must be highly organized with attention to detail and dedication to follow through
- Answer phones and assist Reception staff as needed
- Record Information Accurately
- Maintain Confidentiality of Records and maintains and controls release of information to authorized person(s)
- Scheduling appointments and arranging transportation of residents
- Any and all other duties assigned by Administrator ________________________________________________________________________________________________________
Qualifications:
Required:
- Must have a valid driver’s license
- Must be organized and self-motivated
- Must have ability to work independently
- Must be able to sit, stand or walk 4-6 hours in an 8-hour day
- Must be able to occasionally bend, squat, and twist body during an 8-hour day.
- Must be able to lift a maximum weight of 20# with frequent lifting, carrying, pushing and pulling of objects up to 10#
- Able to read, write and speak English.
- Basic computer knowledge and ability to learn company applications.
Preferred:
- Previous experience in Medical Records for long-term care or skilled nursing experience
