Role DescriptionThis is a full-time, on-site position for a Family Nurse Practitioner serving residents in assisted living and nursing home settings throughout the New York City Metropolitan Area.
Responsibilities include:
- Providing primary care services and managing both chronic and acute conditions
- Conducting admission exams and routine monthly visits
- Collaborating closely with facility staff and interdisciplinary teams
- Helping reduce rehospitalizations through attentive and timely care

Qualifications- Licensed Nurse Practitioner (NP) with a strong foundation in Nursing
- Experience in Primary Care and a solid understanding of general medicine
- Background in Urgent Care, long-term care, or similar settings preferred
- Strong organizational, communication, and decision-making skills
- Ability to work independently while collaborating with medical and facility teams
- Current NP licensure in the state of New York
- Experience in nursing homes, assisted living, or long-term care highly preferred
