Admissions RN
You will:Â
- Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for
ospice care
- Explain hospice services, discuss goals and care with the patient and families
- Obtain medical history, current medications, and symptoms
- Perform comprehensive clinical assessments to
eligibility for hospice based on Medicare/insurance guidelines
- Coordinate the start of
(nursing, aides, equipment, medications.)
- Collaborate with interdisciplinary team
- Maintain
and
timely
documentation

- Graduate of an accredited nursing school with current licensure to practice in state of operation
- One year of hospice nursing experience
, 3 years
Preferred
or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
- Must be a licensed driver with an automobile that is insured
state/or organization requirements and is in good working order
