Job Summary
We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team. The ideal candidate will have a strong background in various nursing specialties, including critical care, geriatrics, and emergency medicine. This role involves providing high-quality patient care in a home setting, ensuring that patients receive the necessary medical attention and support for their recovery.
Responsibilities
- Conduct comprehensive patient assessments to determine health status and care needs.
- Develop and implement individualized care plans based on patient assessments and medical history.
- Administer medications and treatments as prescribed, monitoring for effectiveness and side effects.
- Educate patients and their families about health conditions, treatment plans, and home care techniques.
- Collaborate with interdisciplinary teams to coordinate care and ensure continuity of services.
- Maintain accurate medical records and documentation in compliance with HIPAA regulations.
- Monitor vital signs and perform diagnostic evaluations to track patient progress.
- Provide support for discharge planning, ensuring patients have the resources needed for successful recovery at home.
- Engage in case management activities to facilitate access to necessary services and resources.
Experience
Candidates should possess the following experience or skills:
- Critical care experience in a Level I or Level II trauma center is highly preferred.
- ICU or Emergency Room experience with a focus on acute care management.
- Familiarity with infectious disease care, geriatrics, behavioral health, pediatrics, or post-acute care settings.
- Experience in hospital medicine or outpatient clinic environments is advantageous.
- Strong knowledge of medical terminology, physiology, patient assessment techniques, and discharge planning processes.
