Position Overview:
The primary purpose of your job position is to plan, organize, develop, and direct the overall operation of our facility’s Social Services department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.
Qualifications:
Required: Bachelor’s degree in a Social Science and at least 1 year of experience in a Skilled Nursing facility.
Bachelor’s degree must be in one of the following major fields of study, (without exception):
· Social Work
· Sociology
· Gerontology
· Psychology
· Rehabilitation Counseling

Essential Duties:
- Work as an active participant of the Integrated Departmental Team, (IDT) to achieve the common goals for each resident.
- Ensure milestones of care plans are achieved.
- Take an active role in the admission assessment process.
- Assume responsibility for discharge planning of residents to assure safe and secure outcomes at their destination.
- Plan, develop, organize, implement, evaluate, and direct the social service programs of this facility.
- Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Administrator.
- Develop and implement policies and procedures for the identification of medically related social and emotional needs of the residents.
