Position Summary
The LVN Case Manager supports hospice patients through a combination of field nursing visits and office-based coordination tasks, working closely with the RN Case Manager and interdisciplinary team to ensure quality and compliance in patient care.
Key Responsibilities
- Perform scheduled and PRN (as-needed) nursing visits to hospice patients in their homes, facilities, or RCFEs.
- Provide skilled nursing care such as medication administration, wound care, and symptom management under RN supervision.
- Conduct follow-up visits to assess patient condition and report any changes promptly to the RN Case Manager.
- Educate patients, caregivers, and facility staff on hospice care, comfort measures, and medication routines.
- Participate in care plan updates and IDG (Interdisciplinary Group) meetings as assigned.
- Complete all documentation accurately and in a timely manner per agency policy and regulatory requirements.
- Assist in coordination of supplies, equipment, and communication with pharmacies and DME vendors.
- Support office-based functions including order entry, coordination calls, and visit scheduling as needed.
Qualifications
- Current California LVN License in good standing (Required)
- Minimum 1 year of hospice or home health experience (Preferred)
- Valid California Driver’s License, reliable vehicle, and proof of auto insurance (Required)
- Excellent communication and documentation skills
- Compassionate and professional bedside manner
- Working knowledge of hospice philosophy, symptom management, and Title 22 regulations preferred
Schedule & Work Model
- Monday–Friday, 8:30 AM–5:00 PM (occasional weekend rotation)
- Hybrid model: approximately 50% field / 50% office (varies by census)
- Office located in Los Angeles County
- Mileage reimbursement for travel requirements
Job Type: Full-time
Pay: $42.00 per hour
