Discharge Planner

Supervisory Responsibility: None

Position Qualifications:

Associate degree preferred.

Case Management preferred.

Experience with community resources and local transportation.

Key Responsibilities / Job Accountabilities:

• Initiate discharge planning on the day of admission.
• Based on patient’s needs, develop a continuing plan that promotes their long-term health and compliance.
• Responsible for setting discharge appointments, including group or individual therapy, psychologist or psychiatrist.
• Make arrangements for community resources to facilitate discharge. Provide all needed documentation to other providers to facilitate the continuum of care.
• Ensure all documentation regarding patient discharge planning is clear, concise and timely.
• Act as a liaison between referral sources, patients and counselors.

Additional Skills / Qualifications:

• Strong written and oral communication skills.
• Ability to identify and resolve problems effectively and efficiently.
• Execute various paperwork, like information files processing, and other daily tasks.
• Maintain an efficient, organized work environment.
