Job Summary
Responsible for the provision of care to meet the physical, psychological, and social needs of all patients. This position ensures that each patient is included in healthcare decision-making, as well as works toward creating and sustaining an environment that humanizes and individualizes the care for each patient. This position serves as an advisor for patients and families in decisions about care for immediate needs, as well as aftercare, and provides a range of clinical services to include, but not limited to, assessments of individual psychosocial needs, financial limitations, acute treatment and crisis prevention, as well as post-hospital care.
Position Qualifications
- Bachelor's degree required; Master's degree preferred
- LSW in Ohio required; LISW preferred, or Licensed Professional Counselor (LPC) or Licensed Professional Clinical Counselor (LPCC)
- In-depth knowledge of psychotherapy methods and available community resources
- Understanding of resiliency and recovery in mental illness and substance abuse, and treatment approaches and techniques
Responsibilities
- Provision of social work services and competencies in accordance with licensure standards, regulatory and funding bodies, state, federal and local laws, etc.
- Ensures completion of individual, family, and group assessments with required timelines.
- Provide short-term crisis intervention, as needed.
- Assist patients and families with community resources for post-hospital needs.
- Monitor patient progress toward discharge, ensuring completion of requirements within expected length of stay
- Assist patients and families with securing assistance services, as needed, e.g. Medicaid applications
- Facilitate or assist with psychotherapy groups and ensure documentation compliance with company policy and regulatory bodies, etc.
- Provide individual counseling and ensure documentation compliance with company policy and regulatory bodies, etc.
- Attend treatment team meetings consistent of interdisciplinary team members.
- Performs other duties as assigned.
Working Conditions
Inpatient psychiatric hospital setting that provides acute mental health services. Work is performed in patient care units, therapy rooms, and in treatment areas. The environment may involve exposure to individuals experiencing severe psychiatric symptoms, emotional distress and or behavioral crises. Ability to sit, stand, walk, and engage in patient interactions for extended periods. Occasional lifting and the ability to respond quickly in emergency or crisis situations.
