What are the responsibilities and job description for the Home Care Physical Therapist position at Generational Physical Therapy?
Location: Jericho, NY
Job Type: Part-time
Pay: $60–$120 per hour
Expected Hours: 10–25 hours per week, with additional patients available based on your availability
Make your own schedule while making a difference in the lives of older adults.
We are a small, growing home physical therapy company serving the geriatric population on Long Island. We’re looking for a personable, compassionate Physical Therapist who enjoys connecting with older adults and providing individualized care in their homes.
Currently we have patients centered around the Jericho/Syosset/Plainview area. Most are at one facility in Jericho so commuting between patients does not require driving. We have 30–50 active patients with more patients available to treat every week.
Our referral network is growing with more MDs and hospitals referring patients. We have outgrown the volume that 2–3 PTs can handle and are looking to bring on 2 more qualified PTs.
Whether you’re looking to earn significant supplemental income or build a larger caseload, we can work with your availability. As your available hours increase, we can provide additional patients. Because we are a small independently owned company, you’ll also have opportunities to grow with our company.
What We Offer
• Flexible scheduling: Create your own schedule and arrange patient visits around your availability.
• Competitive pay: Earn $60–$120 per hour.
• A caseload that fits your goals: Start with a manageable number of patients, with more available as your schedule allows.
• Training and support: Receive training on our electronic medical record (EMR) system, patient care expectations, and workflows.
• Room to grow: Build your role as our company expands.
• Personal connections: Work one-on-one with older adults in their homes.
• Clinical autonomy: Manage your patients’ care and progress treatment using your clinical judgment, guided by each patient’s needs and goals.
• Scheduling on your terms: Coordinate visits directly with your patients to create a schedule that works for both of you, with the freedom to manage your own caseload.
Your Role
• Evaluate and treat patients to improve strength, balance, mobility, and daily function.
• Develop individualized treatment plans that support patients’ goals and independence.
• Build positive relationships with patients and their families.
• Educate patients and caregivers on home exercises and safe movement.
• Maintain accurate, timely clinical documentation.
• Communicate patient progress and care needs with our team.
Who We’re Looking For
• A Physical Therapist with a current New York State license.
• A friendly, patient clinician who enjoys working with older adults.
• Strong communication, organization, and time-management skills.
• The ability to work independently and travel to patients’ homes on Long Island.
Apply with your resume and a brief note about your availability and the areas of Long Island you can cover. We look forward to meeting you!
