Responsibilities
- Lead and manage the day-to-day operations of the hospice program, ensuring high-quality patient care delivery in compliance with all regulatory standards.
- Collaborate with interdisciplinary teams including nursing, social work, chaplaincy, and medical staff to develop comprehensive care plans tailored to individual patient needs.
- Oversee patient assessments, discharge planning, and coordination of services across inpatient, outpatient, and post-acute settings.
- Ensure accurate documentation of medical records, patient care activities, and compliance with HIPAA regulations.
- Facilitate intake processes, manage utilization review, and oversee case management to optimize resource use and patient outcomes.
- Maintain a thorough understanding of medical terminology, physiology, and diagnostic evaluation techniques relevant to hospice care.
- Promote a culture of patient service excellence by training staff on best practices in patient assessment, vital signs monitoring, and symptom management.
- Monitor quality metrics related to infection control, emergency response procedures (including trauma center protocols), and behavioral health considerations within hospice services.
Qualifications
- Registered Nurse (RN) licensure with current state certification required.
- Extensive critical care experience including ICU, emergency medicine, or medical-surgical units; experience in Level I or Level II trauma centers is highly desirable.
- Proven background in hospital medicine or post-acute care environments with familiarity in infectious disease care and geriatrics.
- Strong knowledge of HIPAA regulations, medical terminology, discharge planning processes, and electronic medical records systems.
- Demonstrated ability to lead multidisciplinary teams effectively while managing complex cases involving primary care, pediatrics (PICU), or behavioral health needs.
- Experience with utilization management, diagnostic evaluation, intake procedures, and outpatient clinic operations preferred.
- Excellent communication skills with a focus on patient service and compassionate care delivery.
