Key Responsibilities
- Psychosocial Assessment: Conduct initial and ongoing evaluations of patients’ and families’ emotional, social, and financial needs.
- Care Planning: Develop and implement individualized care plans in collaboration with the interdisciplinary team (IDG).
- Counseling & Support: Provide counseling for patients and families, including grief and bereavement support from admission through 13 months post-death.
- Resource Coordination: Assist families in accessing community resources, financial aid, and legal services.
- Crisis Intervention: Offer immediate support during emotional or situational crises.
- Advance Care Planning: Educate and guide families on end-of-life decisions, advance directives, and funeral arrangements.
- Compliance & Documentation: Maintain accurate records and ensure adherence to state and federal hospice regulations.
Job Type: Full-time
Pay: $65,000.00 - $90,000.00 per year
