ROLE DESCRIPTION

The Nurse Practitioner is a full-time, on-site role based in Weakley County responsible for providing psychiatric care to patients across a range of ages and conditions. Day-to-day responsibilities include conducting patient assessments, ordering and interpreting diagnostic tests, and developing evidence-based treatment plans. The Nurse Practitioner will be under direct supervision of a Psychiatrist/Medical Director, and will prescribe medications and therapies within scope of practice, provide preventive care and health education, and coordinate care with physicians, nurses, and other healthcare professionals. The role includes documenting encounters in the medical record, monitoring patient progress, and adjusting care plans as needed to promote optimal outcomes. The Nurse Practitioner is expected to participate in quality improvement efforts, adhere to regulatory and safety standards, and contribute to a respectful, collaborative clinical environment.

 

QUALIFICATIONS

  • Current Nurse Practitioner license in the state of Tennessee and national certification (e.g., FNP, PMHNP, or relevant specialty).
  • Graduation from an accredited Nurse Practitioner program; Master’s or Doctorate in Nursing preferred.
  • Demonstrated clinical skills in patient assessment, diagnosis, treatment planning, and follow-up care.
  • Ability to prescribe and manage medications safely, with strong knowledge of pharmacology and clinical guidelines.
  • Effective communication and interpersonal skills for working with patients, families, and interdisciplinary teams.
  • Strong documentation and electronic health record (EHR) proficiency, with attention to accuracy and confidentiality. Qualifacts/Carelogic preferred.
  • Commitment to evidence-based practice, continuous learning, and participation in quality and safety initiatives.
  • Ability to work independently within scope of practice while collaborating with supervising and consulting clinicians.
  • Experience in primary care, family practice, or a related clinical setting is preferred.
  • Basic life support (BLS) and advanced cardiac life support (ACLS) certifications, or willingness to obtain.
  • Continuing Education:  As required to retain license and registration.

  • Flexible Service Options 
    You have a choice of service options:
    • Two-year full-time clinical practice at an NHSC-approved site.
    • Two-year half-time clinical practice at an NHSC-approved site.
  • Loan Repayment
    You will receive funds to repay your outstanding, qualifying, school loans.

    NHSC loan repayment funds are exempt from federal income and employment taxes.

    Note: If you serve in a private practice, you aren't eligible to practice half-time.

    • Full-time service: Up to $75,000 (primary care providers) or $50,000 (for all providers) for a two-year initial term.
    • Half-time service: Up to $37,500 (primary care providers) or $25,000 (for all providers) for a two-year initial term.
  • Complete Repayment of Qualifying Loans 
    After you complete the initial two-year service contract, you may be eligible to apply for additional loan repayment funds to pay any remaining school loans through one-year continuation service contracts.
    • There is no guarantee that you will receive a continuation contract.

JOB KNOWLEDGE:

  13. Ability to evaluate progress of treatment plan, goals and to make individual modifications as indicated.
  14. Ability to create, compose and edit written materials.
  15. Knowledge of legal and ethical issues related to consumer rights.
  16. Ability to supervise staff and /or students.
  17. Knowledge of community support services.
  18. Knowledge of community health care and vocational services.
  19. Provision of intensive integrated professional mental health services and treatment planning in a clinical or outpatient setting.
  20. Provision of professional leadership in the determination of appropriate medication and therapeutic approaches.
  21. Supervision clinical activities of nurses and students and/or interns.
  22. Establishment and maintenance of clinical records and related documentations for clinical and legal purposes.

 

PRIMARY JOB DUTIES AND RESPONSIBILITIES:

  1. Provides intakes and establishes treatment plans, crisis plans, discharge plans, provides pharmacotherapy to include the prescribing of medications, dosage adjustments, treatment of adverse effects, etc.
  2. Evaluation of the effectiveness of medication in managing the disease state of the consumer.
  3. Evaluation of the medication for adverse side effects, adverse reactions, drug interactions, drug-food interactions and drug disease interactions.
  4. Educating the member about their disease state, the role of medications in managing their disease state, the importance of medication compliance, coping with adverse effects, adverse reactions and interactions which may accompany the medication.
  5. Identifying the need for initiating, continuing, discontinuing or altering the medication.
  6. Performing a complete mental status exam.
  7. Ordering lab work to monitor drug levels as well as adverse reaction/side effects.
  8. Coordination of care with other members of the mental health team, the PCP and other health care providers.
  9. Collects data about patients through interview, case history, tests and or assessment techniques; evaluates data to identify causes in problems and to determine diagnosis and proper therapeutic approach and referral to their providers.
  10. Consults with other providers and related professional and clinical staff and refers to appropriate service as required.
  11. Follows up to assess efficacy of medications and treatment used and makes adjustments and/or changes as indicated.
  12. Maintains medical record documentation to include psychiatric evaluations, CRG/TPG, referrals, medications lists, medical progress notes, lab orders and results, prescriptions, vital signs, flow records, treatment plans, discharge plans and documentation for MOT’s.  Consults with other legal and treatment providers as indicated.
  13. Provides clinical supervision to lower level clinical staff including LPNs, RNs, RNCs, CMAS, and students and/or interns.
  14. Performs miscellaneous job related duties as assigned.
