| QUALIFICATIONS: Education: Minimum: High School Diploma or GED. Experience: Two years preferred POSITION SUMMARY: Plan, coordinate and direct client/residents activity programs utilizing resources of the facilities and community. Recruit, supervise and support staff and volunteers. Conduct activities in accordance with physician orders. Participate in the development and implementation of the assessment process and maintain necessary required records and documentation. The person holding this position is delegated the responsibility for carrying out the assigned duties and responsibilities in accordance with current existing federal and state regulations and established company policies and procedures. JOB KNOWLEDGE: Knowledge of special needs and limitations of the mentally ill; methods and techniques used to include clients/residents and involve them in activities; alert to attitudes and feelings of clients/residents; knowledge of and familiarity with supplies and equipment; familiarity with the community, social agencies, contacts with clubs and groups, and knowledge of sources of materials and local services; understanding and self-discipline to relate sensitively to clients/residents needs and feelings. PROFESSIONAL MEMBERSHIPS: Encouraged to participate in related associations and organizations for the purpose of improving skills and learning new ideas and techniques through sharing with others. OTHER: Enjoy working with the mentally ill, exercise judgment, display empathy, creative skills, imagination, friendliness, resourcefulness, enthusiasm, and have a caring attitude and sincere interest in others. PRIMARY JOB DUTIES AND RESPONSIBILITIES: 1. Organize programs for group and individual activities based on clients/residents individualized needs as identified in clients’/resident’s health-care plans. 2. Responsible for coordinating group and corporate fundraising events. 3. Responsible for maintaining an adequate balance of funds in activities account at all times via donations, fundraisers, etc. 4. Must participate in corporate fundraising events. 5. Maintain a balance of recreational activities including physical, social, religious, divisional, intellectual and creative arts and crafts. 6. Provide motivation for participation in Residents’’ Council and schedule a time and place for meetings. 7. Ensure group activities are scheduled to provide for maximum involvement by interested clients/residents. 8. Select clients/residents for activity participation according to each clients/resident’s activity interest, needs and potential. 9. Coordinate scheduled activities with all group homes. 10. Interview client/resident and families on admission. 11. Communicate clients/residents programs to clients/residents, families, volunteers and facility staff. Communication may occur through posting of a large monthly calendar in centralized locations within the individual homes and circulation of copies of the monthly calendar. 12. Identify and evaluate personal, emotional and environmental concerns that prevent or limit the clients/residents full use of medical, nursing and restorative care. 13. Record continuing progress notes on clients/residents activities and response of client/resident to activity program. 14. Assess spiritual needs and facilities participation in religious activities. 15. Prepare and post Monthly Activity Calendars indicating scheduled activities and times for coming months which reflect interest indentified by the comprehensive assessment, offer activities at hours convenient to the residents (morning, afternoon, some evenings and weekends), reflect the cultural and religious interests of the client/resident population, appeal to both men and women and all age groups living in the facility, take place in a variety of locations (i.e., indoor/outdoor and community-based activities), and include seasonal and special events. 16. Maintain adequate activity supplies and equipment to meet needs and interests of clients/residents, ensure materials are available to clients/residents. 17. Prepare monthly bulletin board indicating future activities, holiday themes or picture review of past activities. 18. Help read incoming mail and address and post outgoing mail upon the clients/residents requests. 19. Give assistance or help in locating and arranging for services of other professionals or agencies to solve problems and needs of clients/residents. 20. Participate regularly in case conferences with other staff to assist in better understanding client/resident and his/her family. 21. Responsible for transportation of clients/residents from one area to another or where activates may occur. 22. Record client’s/resident’s activity participation in client’s/resident’s individual record. Keep group records of participation on scheduled activities. 23. Recruit, orientate and supervise volunteer programs and maintain records of volunteer programs; and communicate facility’s volunteer needs to the community. 24. Participate in community planning related to interests of facility and arrange for the use of community facilities by clients/residents. 25. Participate in in-service program of facilities. 26. Is knowledgeable to provide in-service training for staff regarding activity program, reality orientation, volunteer services, etc. 27. Recognize abnormal signs and symptoms which may signify a physical change in client’s/resident’s condition that could require intervention. Report same to residential manager immediately. 28. Produces monthly newsletter. 29. Maintain professional confidentiality. PHYSICAL REQUIREMENTS: Good physical and mental health. Must be literate, have emotional maturity, integrity, poise, dignity, visual and hearing acuity, vitality and stamina. WORK AREA AND ENVIRONMENT: Variable activities shall be conducted in activity room, dining area, activity office, outdoors, in community and attendance at special events and programs that may be conducted in churches, schools, auditoriums, stadiums, parks and the like. Hours may also vary due to various scheduled activities. SPECIAL DEMANDS: Must have a genuine interest in the mentally ill. Must be willing and able to participate in the on-call rotation which may require working after hours, weekends, and holidays. Position requires an “F” endorsement to transport service recipients, and employee is financially responsible for obtaining the endorsement. Should be patience and tactful, have good listening traits, initiative, imagination, enthusiasm and creativity.
