POSITION SUMMARY:

The primary responsibility of this role is to support seniors and older adults with chronic illnesses in managing their financial benefits and coordinating in-home support services as well as home and community-based care under the Choice for Independence (CFI) Program, in accordance with He-E 805 guidelines for the Choices for Independence Waiver.

This position works Hybrid with mandatory attendance at Gateways’ Nashua office a minimum of two days with 50% of time spend in office or in the field, for monthly staff meetings and other required trainings. Staff are required to be accessible via phone and Microsoft Teams during work hours and maintain an up-to-date calendar within Outlook. During remote work, individual must be able to meet in person if needed and can’t have any other care responsibilities during remote work.

This position is for a caseload of 40 individuals serving clients living in Hillsborough County.

ESSENTIAL JOB FUNCTIONS:

  • Financial Benefits Coordination
  • Assist clients in maintaining CFI benefits by helping them complete annual financial and medical re-certification paperwork.
  • Provide education and resources to clients regarding community resources.
  • Choices For Independence (CFI) Program
  • Conduct initial client comprehensive assessments.
  • Create an initial care plan with the client and submit service requests to the Bureau of Aging and Adult Services (BAAS) for approval.
  • Advocate for clients’ needs and encourage progress toward care plan goals.
  • Update the care plan as needed to reflect changes in clients’ medical condition and or needs.
  • Maintain accurate client records, including progress notes, annual paperwork, and other required forms.
  • Coordinate services with contracted vendor agencies.
  • Ensure timely delivery of quality services as outlined in clients' care plans.
  • Monitor client satisfaction with services through monthly phone contacts and face-to-face visits.
  • Foster communication and professional relationships with the clients’ care team, including managed care organizations and medical professionals when necessary.
  • Stay informed and comply with all relevant state and federal regulations.
  • Identify at-risk clients (abuse, neglect, financial exploitation) and report to the appropriate crisis intervention agency (e.g., BAAS and Adult Protective Services).

DUTIES AND RESPONSIBILITIES:

  • Responsible for promoting health and safety with the organization as defined by Safety Committee/Company policy.

  • Promote and work towards meeting agency goals and objectives as defined in the strategic plan.
  • Compliance with all Gateways Community Services policies and procedures.
  • Complete all necessary required trainings and continue to attend training.
  • Work collaboratively with other staff/departments and programs as necessary.
  • Perform other work-related duties as assigned.
  • Primarily remote workers are responsible for furnishing and establishing a safe, ergonomic, and hazard free home-based office.

QUALIFICATIONS, EDUCATION AND EXPERIENCE:

  • Proficient in Microsoft Windows applications including a working knowledge of Word, Excel, Outlook, along with relational data base knowledge. (Microsoft Access and/or Harmony preferred)
  • Possess reliable cell phone and internet access to provide case management to individuals from a remote (non-Gateways) location.
  • Bachelor’s degree preferred in related field: or equivalent combination of education and/or experience.
  • Three years of prior geriatric casework or related geriatric-based care experience: experience with end-of-life services is preferred.
  • Strong belief in provision of community services for the aging population and people with disabilities.
  • Communicate constructively to management and staff on problems, ideas, or any other insights to assist in defining new best operational practices.
  • Possess reliable transportation, valid driver’s license, and applicable vehicle insurance for business travel.
  • Strong writing skills.

PHYSICAL REQUIREMENTS/WORK ENVIRONMENT

  • Occasional standing
  • Frequent sitting, minimum of 5 hours per day
  • Occasional walking, bending, and twisting of spine
  • Light lifting and carrying, less than 30 lbs.
  • Visual acuity-worker is required to have close visual acuity to perform job functions
  • Climbing Stairs routinely for home visits purposes
  • Pushing and or pulling under 75 lbs. without assistance infrequently
  • Driving routinely and the ability to transport oneself
