Job Title/Position: Home Health Registered Nurse (RN)

Rate Range: $72 - $80 per hour

Part Time (minimum 10 hours per week)

Responsibilities:

• Provide patient-centered care and case management services to individuals in need, and work one on one with patient/ family to improve health and stability.
• Conduct comprehensive assessments of patients' medical conditions and develop individualized care plans
• Collaborate with healthcare professionals to coordinate and facilitate patient care and goals
• Monitor and evaluate patients' progress and adjust care plans as necessary
• Ensure accurate and timely documentation of patient information and medical records including clinical notes, orders, and charting within the Medicare required 48-hour window
• Advocate for patients' needs and rights within the healthcare system
• Educate patients and their families on disease management, treatment options, and available resources

Qualifications:

• Valid Registered Nurse (RN) license in California
• Bachelor's degree in Nursing (BSN) preferred
• Current CPR/ BLS Certification
• Valid California Driver's License
• Customer service experience
• Minimum of 1 year experience with wound care.
• Strong knowledge of medical terminology, physiology, and diagnostic evaluation
• Experience in case management or managed care is highly desirable
• Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals
• Ability to work independently and make sound clinical judgments
• Proficient computer skills for documentation and data management
• Experience with Synergy EMR is preferred but not required
• Experience documenting accurate OASIS data is preferred but not required
• Understanding of the PDGM Medicare model
• Other duties as assigned
