About PACE

Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality, affordable health and support services that preserve and protect their dignity, quality of life, and independence. The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly), affiliated with West Health (www.westhealth.org), is a non-profit that delivers medical, social, and behavioral services to the frail elderly to the North County of San Diego. This PACE center is entrepreneurial in nature and focuses on innovations and bringing new processes to the traditional PACE environment.

Position Summary:

Under the leadership of the Center Manager, the Home Care Coordinator is responsible for the planning, monitoring, and scheduling of in-home services. The Home Care Coordinator will work closely with the Personal Care Supervisor (PCS) ensuring all personal care needs of the participants are met.

Essential Functions:

  • Ensures that home care schedules are maintained, accurate, and participants are allocated enough time/services.
  • Coordinates care through staffing with contracted home care providers as well as Personal Care Attendants (internal staff).
  • Coordinates visits, considering the qualifications of the Personal Care Attendant and the participants level of care.
  • Re-assigns daily visits if a staff member cannot conduct visits as scheduled. Informs Personal Care Supervisor (PCS) when visits cannot be rescheduled.
  • Always acts proactively in modifying the schedule to ensure accuracy. Notifies participant and/or primary caregiver in advance of changes to the schedule.
  • Participates as a member of the Interdisciplinary Team (IDT) and in initial, semi-annual, unscheduled, and annual assessments.
  • Attends morning updates and reports changes in participants' status to appropriate staff regularly.
  • Performs homecare assessments for Service Delivery Requests (SDR) and updates in the EMR per regulatory guidelines.
  • In collaboration with the PCS and the IDT, may be asked to participate in the initial enrollment meetings with family members and others.
  • Submits authorization requests to home care agencies as required.
  • Has the ability to fill in as PCA when the need arises or in emergent situations.
  • In conjunction with the PCS, assures that documentation for home care is accurate, current, and entered in the EMR timely.
  • Maintains confidentiality regarding participant, staff, contractor, and organizational information.

Non-Essential Job Functions:

  • Attend and participate in staff meetings, in-services, projects, and committees as assigned.
  • Adhere to and support the center's policies, practices, and procedures.
  • Accept assigned duties in a cooperative manner and perform all other related duties as assigned.
  • Be flexible in schedule of hours worked.
  • Will require use of personal vehicle.

Education and Certification:

  • High school diploma/GED required.
  • LVN/LPN preferred.
  • Current driver license.
  • Proof of auto insurance.
  • Clean driving record.
  • BLS and CPR Certification.

Skills and Experience:

  • Minimum of two (2) years of demonstrated successful experience in home care; prefer in-home care management experience.
  • Minimum of one (1) year of documented experience working with a frail or elderly population.
  • Experience in the provision of person-centered care, including assessment skills, personal care, and health education.
  • Working knowledge of physical, mental, spiritual, and social needs of the frail elderly and their families.
  • Effective time management skills.
  • Knowledge of utilization review, quality improvement and cost containment systems.
  • Ability to work with ethnically diverse populations.
  • Ability to apply creative problem-solving skills to the complex issues in home care.
  • Ability to work with interdisciplinary teams.
  • Strong organizational skills, flexible, resourceful, and energetic.
  • Effective oral and written communication skills.

 

Medical Clearance:

Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
