This is a full-time, on-site role for an MDS Coordinator based in our Hudson Springs Nursing and Rehabilitation location. The MDS Coordinator will oversee and coordinate the accurate and timely completion of Minimum Data Set (MDS) assessments and care plans. Primary responsibilities include conducting resident assessments, developing care plans, managing care documentation, and ensuring compliance with state and federal regulations. The MDS Coordinator will collaborate with healthcare staff to ensure quality care for residents.

Qualifications
- Valid RN or LPN licensure
- Proficiency in creating and managing Care Plans
- Experience with MDS assessments and compliance
- Strong understanding of Long Term Care Nursing principles

- Ability to work cooperatively within a multidisciplinary team
- Prior experience in long-term care or skilled nursing environments is a requirement
