Position Overview:
We are seeking an experienced referral coordinator with a certification as a Community Health Worker (CHW). This person should have strong knowledge of IEHP and TRICARE referral systems and quality measures. The ideal candidate is organized, proactive, and detail-oriented, with excellent communication skills and a passion for improving healthcare access and outcomes for children.
Key Responsibilities:
Coordinate and track all patient referrals, including both IEHP and TRICARE, ensuring timely scheduling, follow-up, and documentation.
Complete and submit billing documentation for visits and services related to IEHP and TRICARE programs.
Maintain an active NPI number and accurately document and bill for patient assistance visits as required.
Monitor and report on IEHP quality metrics, assisting with practice improvement initiatives.
Support the team in preparation for periodic IEHP audits and ensure documentation accuracy and compliance.
Communicate effectively with families, providers, and health plan representatives to ensure continuity of care with appropriate documentation and associated billing.
Maintain accurate records within the EMR and assist in data collection and quality reporting.
Qualifications:
Certified Community Health Worker (CHW) — REQUIRED
Active or eligible NPI number is preferred.
Demonstrated experience with IEHP and TRICARE referral and billing systems.
Prior experience in pediatrics or community health strongly preferred.
Strong organizational and computer skills, including proficiency with electronic health records (Office Practicum experience a plus).
Excellent communication, follow-through, and teamwork abilities.
High school diploma or equivalent required; associate’s degree or higher preferred.
