Position Overview
We are seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to deliver high-quality nursing care and provide direct supervision to nursing assistants. In this role, you will support our residents' overall health and well-being while ensuring compliance with facility standards and state/federal regulations.
Key Responsibilities
- Provide direct nursing care to residents in accordance with care plans and physician orders
- Supervise Certified Nursing Assistants (CNAs) during daily care activities
- Administer medications and treatments as prescribed
- Monitor residents' conditions and report changes to the RN or physician
- Participate in care planning, charting, and documentation
- Assist in admissions, discharges, and resident assessments
- Maintain a clean, safe, and supportive environment
- Communicate effectively with residents, families, and interdisciplinary teams
- Assist with wound care, Braden assessments, and skin condition monitoring
- Maintain accurate medication and narcotic records
- Ensure compliance with infection control and safety protocols
- Support emergency preparedness and evacuation procedures
- Valid New York State LPN license (required)
- Graduate of an approved LPN program
- Prior experience in long-term care or rehabilitation setting preferred
- Strong clinical skills and a solid understanding of nursing practices
- Excellent communication, organization, and supervisory skills
