Job Summary:
To work within the DPH/BSAS programs team model, establishing open communication, support, and accountability among workers and consumers. S/he is accountable to the Program Director, who is responsible for providing supervision and training. The Intake Staff is one who actively promotes the philosophy of Gandara Center, and maintains a high level of commitment to the program assigned to and the agency.
Duties and Responsibilities:
- To maintain / coordinate potential participant referrals on program spreadsheet and State database.
- To screen potential clients guided by program admission criteria and ASAM criteria
- To conduct the interview for admission to the RRS treatment program
- To accurately update program census sheets
- To review Resident Handbook to new resident upon admission to the house and coordinate an orientation date/time with House Manager assigned that task.
- To coordinate resident provider messages, provider apt, and resident transportation needs to appointments
- To compile appropriate documentation for resident file binder
- Share all expectations of program Direct Care I/RSS position.
Administrative Responsibilities/Record Keeping
- To attend scheduled staff meetings.
- To screen all incoming calls, determine their nature, and forward information through proper channels.
- To maintain client records in accordance with standard format and timetables.
- To attend scheduled supervisory meetings with Program Director/Clinical Supervisor
- To maintain assigned financial record keeping, and submits required reports accurately and in a timely manner.
- To attend all trainings as required by funding source and the Agency
- To organize residents for Food Stamp withdrawal
- To become approved for BSAS EIM/ESM data entry
- To input new resident into BSAS EIM / ESM electronic database
- To input new resident information into Gandara EHR and activate record
- To accurately maintain and update program resident census
- To outreach to potential residents and referral source for referral follow-up
- Community liaison; responsible for attending community meetings
- Holyoke safe neighborhood initiative, Springfield’ New North Citizens Council /C3 Community Policing meeting both meetings will connect said programs to resources, keep us in the now of what is going on around the community as in activities and safety issues concerns, and other recovery resources
- Responsible for requesting and confirming PCP / BH provider resident documentation, to aid us in complying with insurance request of physicals and in obtaining BH/MH DX on residents who do not have a therapist out of Gandara.
- Understanding of program and third party biller admission criteria
- Ability to navigate EHR’s and web linked software and applications
- Ability to work with population in recovery.
- Ability to structure and organize job responsibilities with minimal supervision.
- Demonstrates good verbal and written communication skills.
- Bilingual (Spanish/English)

Minimum Qualifications:
- High School/GED 2 yrs. Experience with SUD/MH consumers
- Associates Degree 2yrs experience with SUD/MH consumers
- Bachelor’s degree 1-2 years’ experience preferred with SUD/MH consumers
- Demonstrated strong communication and interpersonal skill, including conflict resolution.
- Demonstrated organizational skill, able to multi-task.
- Demonstrated ability to lead through example.
- Must have valid driver’s license
- Must meet Agency requirements for driving history records.
- Must have availability of an automobile to transport clients or perform other supportive services as necessary.
