To work within the Residential Rehabilitation Services (RRS) to meet the American Society for Addiction Medicine (ASAM) definition for Level 3.1, which supports Members’ recovery from addiction and moderate mental health conditions while reintegrating into the community and returning to social, vocation/employment, and/or educational roles. The Recovery Resource Coordinator will identify community-based resources for referral, provide orientation and aftercare service coordination, and psycho-education services to residents transitioning to community and their family members and to create and maintain an alumni group.
Duties and Responsibilities:
- To maintain supportive contact with residents to continually assess their individual needs, especially during initial period of residency and during stressful and crisis situations.
- To provide advocacy/case management services whenever appropriate for each resident.
- To develop strong, supportive relationships with the clients.
- To develop, cultivate and manage a positive culture and milieu.
- To provide for client care and safety and crisis intervention when necessary.
- To notify Program Director immediately in the event of a client crisis, suspected illegal activity or any behavior constituting an incident in the program and handle these situations in the Director’s absence.
- To interact with clients in order to assess needs, provide information, discuss problems, set limits and encourage growth and development and to monitor each client’s progress.
- To provide residents with an understanding of house routines, rules, and responsibilities (including curfew routines, sleeping quarters, maintenance and household chores, etc.).
- To develop and maintain a working knowledge of local community resources and social service agencies and develop positive and sustaining working relationships with them.
- To maintain updated clinical notes and reporting in accordance to established record keeping procedures.
- To participate in all case consultation meetings, staff meetings and case conferences, as appropriate.
- To provide orientation of services to prospective clients, family, and community groups.
- To facilitate the Family Orientation Group, the Consumer Advisory Committee, and the Alumni Support Group, and maintain minutes thereof.
- To provide recommendations for discharge planning and aftercare services.
- To adhere to program and agency policies, procedures and protocols.
- To design and implement an orientation and aftercare process.
- To participate in all case consultation, staff meetings, trainings and case conferences, as appropriate.
- To maintain assigned financial or other types of record keeping, and submit required reports accurately and in a timely manner.
- To assist Program Director in public relations duties, including but not limited to scheduled tours and screening of incoming calls.
- To attend scheduled supervisory meetings with Program Director.
- To follow through on supervisory directives as provided by Program Director.
- To perform other duties as assigned by the Program Director.
- To participate in an on-call rotation and respond to after-hour crisis management
- Associate’s Degree and/or three years’ experience with target population.
- Ability to structure and organize job responsibilities with minimal supervision.
- Demonstrates good math, verbal and written communication skills.
- Displays excellent supervisory and delegation skills.
- Bi-lingual and/or bi-cultural preferred.
- Physical and mental demands are located at the end of this job description
- Must have valid driver’s license, auto insurance, ability to meet agency’s driving requirements, and ability to assume travel assignments related to work.
