The case manager is accountable to the Program Director/Clinical Supervisor, who is responsible for providing supervision and training. The Case Manager staff is one who actively promotes the philosophy of Gándara Center, and maintains a high level of commitment to the program.

The case manager is accountable to the Program Director/Clinical Supervisor, who is responsible for providing supervision and training. The Case Manager staff is one who actively promotes the philosophy of Gándara Center, and maintains a high level of commitment to the program.
Duties and Responsibilities:
- To develop strong, supportive relationships with the clients, and provide for client care and safety.
- To complete all clinical documentation and casework for assigned residents: including but not limited to: adult comprehensive assessment, individual action plans, group progress notes, weekly progress notes, discharge summary, and transition/discharge/aftercare planning.
- To maintain updated clinical notes and reporting in accordance to established record keeping procedures.
- To facilitate daily evidence based psychoeducation groups and other recovery based groups.
- To administer and complete the EIM/ESM forms and adhere to funding sources requirements.
- To maintain direct supportive contact with residents to continually assess their individual needs, provide information, discuss problems, set limits and encourage growth and development.
- To be capable of assuming enhanced responsibilities for planning and overseeing on and off unit activities.
- To develop and maintain a working relationship and knowledge of other social service agencies in the community.
- Associates Degree 2yrs experience
- Bachelor’s degree 1-2 years’ experience preferred.
- Demonstrated strong communication and interpersonal skill, including conflict resolution.
- Experience providing substance abuse services
- Demonstrated organizational skill, able to multi-task.
- Demonstrated ability to lead through example.
- Must have valid driver’s license and must meet Agency requirements for driving history records.
- Must have availability of an automobile to transport clients or perform other supportive services as necessary.
Salary : $23 - $26
