HOSPICE REGISTERED NURSE CASE MANAGER

POSITION OVERVIEW
The Hospice RN Case Manager will deliver expert nursing care and comprehensive case management services to hospice patients in Denton County.
Duties include assessing patient needs, managing symptoms, coordinating care plans, and offering education and emotional support throughout the hospice journey.
KEY RESPONSIBILITIES

Deliver skilled nursing care based on individual care plans.

Conduct patient assessments and monitor changes in condition.

Coordinate clinical care for assigned hospice patients.

Administer medications and treatments as prescribed.

Provide pain and symptom management effectively.

Educate patients, families, and caregivers on medications, symptom management, and safety.

Collaborate with the interdisciplinary team to ensure patient-centered care.

Communicate changes in patient condition promptly.

Respond to urgent patient needs and provide crisis intervention when needed.

Maintain accurate clinical documentation as per regulations.

Participate in on-call duties.

Travel throughout Denton County.

REQUIRED QUALIFICATIONS

Current Texas RN license.

Minimum one year of hospice nursing experience.

Reliable transportation and automobile insurance.

Ability to travel and participate in on-call rotation.
SCHEDULE
-
Full-time, Monday to Friday with on-call responsibilities.

Local travel within Denton County.
COMPENSATION

Base salary of $85,000–$90,000 annually.

Mileage reimbursement and cell phone allowance provided.
