Gabby Health is seeking a Montana-licensed physician to provide on-site medical coverage at skilled nursing facilities in the Hamilton, MT area — just 2 days per month. This is a flexible, low-commitment role designed for a physician who wants meaningful patient impact without a full-time schedule.
Rural Montana communities face a real shortage of qualified medical professionals. This role directly addresses that need, with the full support of the Gabby Health clinical team behind you between visits.

What You'll Do
- Conduct in-person visits at SNF facilities in the Hamilton, MT area (2 days/month)
- Perform H&Ps, follow-up visits, and medical management for SNF residents
- Partner with Gabby Health's telehealth team to ensure continuity between in-person visits
- Document encounters in accordance with CMS and facility standards
Qualifications
- Active, unrestricted Montana physician license (MD or DO) — required
- DEA registration current and in good standing
- Internal Medicine, Family Medicine, or Geriatrics background preferred
- SNF or post-acute experience strongly preferred
- Familiarity with PointClickCare (PCC) a plus
