The Registered Nurse (RN) supports individuals with severe and persistent mental illness by providing health assessments, education, and coordination of medical care. The RN collaborates with community providers to monitor physical health, manage medication side effects, and ensure access to necessary medical services. They also contribute to wellness planning and implementation alongside the ACT team.

Essential Functions
- Attend daily team meetings and participate in the development and implementation of interventions designed to address barriers to community integration.
- Provide medication administration, and monitoring to participants.
- Assisting participants with medication adherence:?developing strategies and support systems to help clients take their medication as prescribed.
- Provide basic health screenings.
- Communicate and coordinate with PCP, medical specialists, hospitals and rehabilitation programs regarding medical recommendations.
- Consult with the ACT team regarding the medical recommendations of community health agencies to ascertain the capacity of the team and the participants to meet the discharge recommendations.
- Coordinate with community providers to obtain medical support such as caregivers, medical supply ordering and maintenance.
- Provide in-home skills training to participants to monitor and effectively manage medical conditions such as diabetes, COPD and Hepatitis.
- Provide support to the psychiatric nurse practitioner in the administration of injectable medication.
- Provide individual and group skills training regarding nutrition, medication, and incorporating physical health as a component of recovery.
- Work with health team support staff to ensure medications are ordered, stored, and documented per pharmacy and state regulations.?
- Attend and participate in ACT Team training and meetings.
- Comply with Laurel Hill Center policies and procedures in addition to Medicaid, ISSR and HIPAA standards.
- Meet Universal Job Expectations.
- ACT team members are required to work no less than one evening or weekend shift per week and will be part of an on-call rotation for the crisis line in case an ACT participant calls. Crisis line response may include community/in person response.
- Nursing degree from an accredited college or program and RN license
- QMHA credential expected upon hire
- Experience with individuals who have been diagnosed with a psychiatric illness or coursework in psychology
- Flexibility, strong verbal and written communication skills, problem solving and negotiation skills
- Ability to lift up to 25 lbs. and navigate stairs
- Current Driver’s License and a good driving record
- Documentation of successful completion of a background and fingerprint check as specified by Oregon law, as well as a urinalysis drug screen
