Personal Wellness Support Lead (In Home Day Program)

Our story began on January 6, 1964 in Lafayette, California with a small group of young adults, their parents and the founding Director, Helen Young. For over 60 years, Futures continues to serve the same population now, in several counties in California.

Under the supervision of the Program Coordinator and Program Supervisor, the Personal Wellness Support Lead , will play a vital role in enhancing the quality of life for adults with disabilities in our home-based day program. Our Personal Wellness Support Leads provide support to participants in their home and in their local community. Individuals in our In Home Program are unable to attend traditional day programs due to their health. Personal Wellness Support Leads work directly with the participant and their care teams to develop an individualized activity calendars, ensuring outings and activities are person centered. The ideal candidate is creative, motivated, responsible, adaptable, and attentive.

Duties:

• Works directly with the participants to develop meaningful Individual Service Plan goals for all Annual Reports.
• Conducts Semi-Annual and Annual Meetings with participants, their care team, and Regional Center of the East Bay.
• Work independently and have some flexibility to adapt to changes.
• Ability to analyze problems, situations, and alternatives to make effective decisions.
• Possess excellent interpersonal and customer services skills.
• Have the physical stamina and the patience to care for your participant.
• Comply with the Dependent Adult and Elder Abuse Requirements as stipulated by law.

Qualifications:

• Experience in caregiving or related fields, particularly with individuals with developmental disabilities.
• Strong interpersonal and communication skills for interacting with adults and their families in a respectful and supportive manner.
• Ability to work independently.
• Compassionate and patient demeanor with excellent interpersonal skills to build rapport with clients and their support teams.
• Organization skills to manage time effectively and document participant case notes.

Job Requirements:

• At least 18 years of age.
• Authorized to work in U.S.
• High school diploma or GED
• Two (2) years or more of related work experience, preferably
• Possess a valid CA driver’s license, insurable driving record, and evidence of auto insurance. MUST BE WILLING TO USE PERSONAL VEHICLE TO TRAVEL TO AND FROM THE PARTICIPANTS HOMES. MILEAGE REIMBURSEMENT IS PROVIDED.
• Knowledgeable of computers and software use, MS Office (Word, Excel, Outlook) and G Suite Workplace.
• Good written and verbal communication skills.
• LiveScan criminal record clearance (or exemption) prior to start.
• Health Screening and TB Test clearance prior to start.
• Physical stamina and ability to meet the physical requirements necessary to perform the assigned duties safely and effectively.

Great if you have:

• Knowledgeable of Lanterman Act regulations, Regional Center guidelines, and State of California Licensing regulations.
• Certified in First Aid/CPR/AED by a qualified instructor.
