Job Summary:
As a Direct Support Professional at Futures Community Support Services, you’ll provide daily support, supervision, and guidance to individuals with disabilities living in a group home. You’ll help with personal care, medication administration, and community integration while promoting independence, dignity, and choice.
Key Responsibilities:

Support individuals with daily living skills such as personal care, cooking, and housekeeping

Implement and document Individual Service Plans (ISPs)

Administer medications and maintain Medication Administration Certification

Encourage independence and respect individual choices
-
Provide emotional support, advocacy, and community engagement
-
Complete required documentation accurately in the agency’s electronic health record system

Attend required trainings and staff meetings
-
Promote a positive, inclusive, and safe environment
Job Requirements:
- Possess a valid driver's license for at least 3 years
- License must be valid in PA or NY
- Satisfactory DMV Background Check
- Able to work on weekends, holidays, and during inclement weather.
Physical Requirements:

Lift and transfer up to 100 lbs. (with or without assistance)

Perform repetitive motions, bending, reaching, and wheelchair support as needed

Maintain awareness and quick response to ensure safety
Working Conditions:

Positions available for evenings, overnights, weekends, and holidays

Support individuals who may have co-occurring mental health needs or challenging behaviors

Must complete a pre-employment physical and Mantoux TB test upon hire.
