Facility Information:
Our client is hiring a Direct Support Professional I (DSP I) to provide support to adults with disabilities. As a Direct Support Professional I (DSP I), you will be responsible for the implementation of Individual Service Plans (ISP) that will lead to or continue to promote independence and personal skills, well assisting in providing a safe and secure home environment for persons supported in our client’s Independent and Community Living Services program.
DSP I Essential Responsibilities:
Facility Information:
- Our facilities are Intermediate Care Facilities for individuals with disabilities (ICF/DD-H and ICF/DD-N)
- Caregiver (DSP) to people supported ratio is usually 1:3, sometimes as high as 1:1 support
- Nursing caseload typically ranges from 6-15 residents
- Our home sizes are small, including 6-bed and 15-bed facilities
- High-acuity care is limited or prohibited by facility regulations
- Our Immediate Care Facilities provide wound care per licensing requirements, ranging from simple wounds to a maximum of stage I pressure ulcers
- Hospice care is very limited and is provided collaboratively with professional hospice care providers who come to our homes

As a Direct Support Professional I (DSP I), you will be responsible for the implementation of Individual Service Plans (ISP) that will lead to or continue to promote independence and personal skills, well assisting in providing a safe and secure home environment for persons supported in our client’s Independent and Community Living Services program.
DSP I Essential Responsibilities:
- Provides assistance with daily living skills in accordance with the persons supported schedule and Individual Service Plan (ISP)
- Assists with daily needs/tasks within persons supported home
- Prompts and provides assistance with medication management, as needed
- Assists persons supported in all aspects of personal care, including hygiene
- Assists persons supported in weight bearing and non-weight bearing transfers, as needed
- Complies with all Momentum policies, procedures, and regulations
- Performs other duties as assigned to meet departments and/or business needs
- Must be 18 years of age or older
- Valid and current proof of legal right to drive in California; required
- High school diploma or equivalent; required
- Must have reliable phone/access to phone (monthly stipend)
- Minimum of one (1) year experience working with people with developmental and/or other disabilities; preferred
- Must be able to read, write, comprehend, and communicate effectively in English
- Successfully pass all post-offer, pre-employment screening requirements, including criminal background check, drug screening, and provide proof of eligibility to work in the United States
- Ability to run, push, pull, bend, squat, kneel, lift up to 50 lbs., and transfer individuals
- Use of personal vehicle for assignments throughout service areas not normally assigned; highly preferred
- Must be available to travel throughout Los Angeles and Orange Counties, as needed (mileage reimbursed)
