Qualifications
- Active RN license in Ohio
- Hospice experience preferred; home health or related post-acute experience considered
- Strong clinical assessment and care planning skills
- Ability to manage a defined caseload independently
- Strong communication skills with patients, families, and interdisciplinary teams
- Willingness to participate in an on-call rotation
Responsibilities
- Manage and coordinate hospice care for patients within a defined Cleveland territory
- Conduct comprehensive assessments and develop individualized care plans
- Collaborate with physicians and interdisciplinary team members to ensure quality care
- Educate patients and families regarding comfort-focused treatment and support
- Maintain accurate and compliant clinical documentation
- Participate in on-call rotation as scheduled
