Role Description
This is a full-time, on-site role based in Bluffton, Ohio, for a CDCA Case Manager at Full Circle Recovery Services. The Case Manager will be responsible for conducting client assessments, developing personalized care plans, and coordinating resources to assist clients in their journey to recovery. Additional responsibilities include maintaining client records, providing support and guidance, collaborating with healthcare providers and counselors, and monitoring progress toward treatment goals while ensuring a compassionate and respectful approach to client care.
Qualifications
- Proficiency in case management, ability to create individualized care plans, and strong organizational skills
- Knowledge of addiction treatment and substance use disorders, and the ability to apply evidence-based practices
- Strong interpersonal communication, active listening, and empathy skills for building trust with clients
- Capability to collaborate effectively within a multidisciplinary team including physicians, counselors, and other healthcare professionals
- Solid documentation skills and familiarity with maintaining detailed, confidential client records
- Certified Chemical Dependency Counselor Assistant (CDCA) accreditation or willingness to obtain certification, as required
- Ability to work professionally in an on-site setting in Bluffton, Ohio
- Prior experience in addiction recovery services is preferred but not required
