Summary:
The Standard Outpatient Program (SOP) CDCA Case Manager provides direct care and standard outpatient care in a mobile setting to clients of Full Circle Recovery Services.
Schedule:
Mon-Tue/Thur-Fri
8am-6pm
Location: Covers Fairfield and Middleton, Ohio locations
Key Responsibilities:
- Assist clients in adhering to treatment plans by scheduling appointments, arranging transportation, and offering general support.
- Collaborate with counselors, providers, and nurses to plan and coordinate treatment, leveraging case management experience and understanding of patient needs.
- Provide individual and group counseling to clients dealing with substance abuse, mental and physical illness, poverty, unemployment, or abuse.
- Educate clients and community members about mental and physical illnesses, abuse, medications, and available community resources.
- Enhance case management skills through reading current literature, conducting social research, and attending relevant seminars, workshops, and classes.
- Interview clients, review records, and consult with other professionals to assess client conditions.
- Modify treatment plans based on client progress and status updates.
- Monitor, evaluate, and document client progress toward treatment goals.
- Refer clients or families to community resources for housing, treatment, or other services and follow up to ensure effective service delivery.
- Display empathy and sensitivity to the current needs of clients.
- Travel and transport clients as necessary to fulfill job responsibilities.
Qualifications and Education Requirements:
- Minimum CDCA licensure required.
- Must have reliable transportation.
- Experience working with individuals with substance abuse issues.
- Strong relationships with community organizations and services.
- Ability to follow written and oral instructions and comprehend human behavior.
- Maintain professionalism when addressing emotionally intense issues or situations.
- Capable of decision-making with limited information.
- Communicate effectively with individuals from diverse backgrounds.
- This position may require the use of a Company or personal vehicle. Candidates must have a valid driver’s license and possess the ability to comply with agency auto insurance policy
