Responsibilities:
As part of the Fukuji & Lum ohana, Physical Therapists carry their own caseload and engage in mentoring activities (receiving and providing) and monthly in-service opportunities. Patient care is our priority. Our Kaneohe campus specializes in Workers' Compensation rehabilitation from initial Physical Therapy through Work Conditioning and Work Hardening. We also provide Functional Capacity Evaluations and Aquatic Therapy at this location.

Maintain Open and Honest Communication with the Clinic Advisor
- Provide transparent feedback to the Clinical Advisor (CA) about clinic operations, patient care, workflow improvements, and your needs as an employee.
- Participate in daily huddles to discuss obstacles, review clinic workflows, and resolve open issues.
- Communicate any issues or concerns promptly for discussion and resolution.
- Be open to quick bursts and other management tools implemented by the CA.
Deliver Patient Care Within Scope of the Practice Act & PPM
- Act as the primary PT for patient caseload and responsible for directing patient care.
- Evaluate patients with orthopedic and neurological diagnoses.
- Create, implement, and adjust appropriate Plans of Care to patients’ ongoing needs.
- Create a Home Exercise Program on the first visit and update it on subsequent visits.
- Instruct primary caregivers on necessary medical information to allow optimal patient outcomes.
- Communicate with and supervise PTAs regarding Plans of Care and any necessary adjustments.
Perform Documentation to F&L Standards
- Maintain accurate and compliant documentation per F&L standards.
- Complete all documentation for IE, DN, PN, and DC within 48 hours.
- Respond to Prompt EMR task requests and interoffice communications daily.
- Review Prediction Health Audit Score monthly to improve documentation efficiency and compliance.
- Submit insurance authorizations as determined by F&L procedures.
- Update Case Overview per F&L procedures on the Dashboard weekly.
Enhance Professional Growth & Development
- Participate in continuing education as the state board of Physical Therapy requires and in collaboration with the mentor’s recommendations for continued growth.
- Participate in monthly staff meetings and activities to develop interpersonal skills, communication, and collaboration.
- Actively participate in LCWs (in-clinic training) for professional growth and development.
- Actively participate in the company’s public relations/marketing efforts as needed.
Support the Company’s Professional Practice Model
- Apply a whole-person approach to care by addressing physical, emotional, and social aspects of patient treatment.
- Meet regularly with a Clinical Mentor and your CA to progress professional development and growth path.
- For candidates with >3 years of experience, act as a Clinical Mentor for a newer professional.
- Supervise a volunteer or student yearly to develop mentorship and organizational skills.
- Uphold the values and goals of the organization through quality care and teamwork.
The Kaneohe Clinic is open Monday through Friday and your hours would be 7:15am to 6:30pm.
