Duties
- Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ vital signs and observe for any changes in health or behavior, reporting findings accurately
- Administer medications following care plans and HIPAA regulations to ensure safety and compliance
- Prepare nutritious meals and assist with meal planning to meet dietary requirements
- Support with light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment
- Use specialized equipment like Hoyer lifts or other assistive devices to safely transfer residents with mobility challenges
- Document care activities, update care plans, and communicate effectively with healthcare teams and family members
Skills
- Proven experience in home health, assisted living, nursing homes, or group home settings
- Knowledge of patient observation techniques and monitoring vital signs accurately
- Strong understanding of dementia, Alzheimer’s care, memory care, and working with individuals with developmental disabilities or autism
- Certified in CPR and First Aid; familiarity with HIPAA regulations for patient privacy
- Ability to perform heavy lifting safely and operate assistive devices such as Hoyer lifts
- Excellent communication skills for working with patients, families, and healthcare professionals
- Experience in medication administration, care planning, and supporting patients through complex health conditions such as hospice or long-term care needs
