Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
• Monitor patient health by taking vital signs, observing for changes in condition, and reporting findings accurately while respecting HIPAA privacy standards
• Support medication administration and care plan implementation under supervision, ensuring safety and compliance with healthcare protocols
• Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption to promote proper nutrition
• Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning client areas, laundry, and sanitizing equipment
• Provide companionship and emotional support to reduce feelings of isolation, especially for clients with dementia or Alzheimer’s disease
• Document care activities thoroughly in accordance with agency policies and state regulations to ensure continuity of care

Qualifications

• Prior experience working in group homes, assisted living facilities, nursing homes, or home & community care settings is highly preferred
• Knowledge of patient observation techniques, medication procedures, and vital signs monitoring is essential
• Certification in CPR and First Aid is required; additional certifications such as HIPAA training are a plus
• Experience with specialized care such as Alzheimer's/dementia care, autism support, or hospice care is advantageous
• Ability to perform heavy lifting safely and use assistive devices like Hoyer lifts or resident lifts when necessary
• Strong interpersonal skills with a compassionate attitude toward individuals with disabilities or health challenges
• High school diploma or equivalent; additional training in social work or nursing-related fields is beneficial
