Home Health - Full Time - RN Case Manager
Location: Everman, TX, 76140
We are seeking a highly skilled and experienced Home Health - FT - RN Case Manager to join our team in Highland Park, TX. The ideal candidate will have a strong background in home health care and be able to provide exceptional care to our patients.
Responsibilities:
- Assess patient needs and develop individualized care plans
- Coordinate and manage patient care with other healthcare professionals
- Provide skilled nursing care to patients in their homes
- Educate patients and their families on proper care techniques and disease management
- Monitor patient progress and adjust care plans as needed
- Document patient care and progress in electronic medical records
- Communicate with physicians and other healthcare professionals regarding patient care
- Ensure compliance with all state and federal regulations
Requirements:
- Current RN license in the state of Texas
- Minimum of 2 years of experience in home health care
- Strong clinical skills and knowledge of disease management
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
- Experience with electronic medical records
- Valid driver's license and reliable transportation
