Job Summary:
We are seeking a skilled Home Health -Physical Therapist (PT) - PRN - Per Visit - Home Health to join our team in Dallas, TX. The ideal candidate will have experience in providing physical therapy services to patients in their homes. As a Home Health - Physical Therapist (PT), you will be responsible for assessing patients' needs, developing treatment plans, and providing therapy services to help patients regain their mobility and independence.
Responsibilities:
- Conduct initial assessments of patients' physical abilities and develop treatment plans
- Provide therapy services to patients in their homes
- Monitor patients' progress and adjust treatment plans as needed
- Communicate with patients' physicians and other healthcare professionals to coordinate care
- Document patients' progress and maintain accurate records
- Provide education to patients and their families on how to manage their conditions and prevent further injury
Requirements:
- Current license to practice as a Physical Therapist in the state of Texas
- Minimum of 1 year of experience in home health
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and critical thinking skills
- Valid driver's license and reliable transportation
