Home Health – Intake Coordinator (Hybrid)

Job Summary:
Primary function is to facilitate the referral intake process and coordinate care with the interdisciplinary team, the patient, their family, and the referral source.
Job Qualifications:
Education: Graduate of an accredited school of vocational/practical nursing.
Licensure: Active, state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state. Current driver’s license.
Experience: One year of experience as a licensed vocational/practical nurse; home health experience preferred.
Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.
Transportation: Reliable transportation and valid auto liability insurance.
Environmental and Working Conditions:
Works in a routine office environment. Noise level may be moderately high. The ability to work a flexible schedule and extended hours. The ability to travel locally with some exposure to inclement weather. Possible exposure to toxic materials, toner, etc.
Physical and Mental Effort:
Prolonged sitting and some standing required. The occasional need to lift, pull, carry, and push items weighing up to 50 pounds. Frequent need to stoop, kneel, and reach while accessing files. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions. Requires hand-eye coordination and manual dexterity. Requires excellent problem-solving skills.
