Job Summary
The Home Health - Registered Nurse will provide skilled nursing care to patients in their homes. They will assess patient needs, develop care plans, and implement and evaluate nursing care. The Home Health - Registered Nurse will also educate patients and their families on proper healthcare practices and disease prevention.
Responsibilities
- Assess patient needs and develop care plans
- Implement and evaluate nursing care
- Administer medications and treatments as prescribed by physicians
- Monitor and document patient progress
- Provide education to patients and their families on proper healthcare practices and disease prevention
- Collaborate with other healthcare professionals to ensure quality patient care
- Adhere to all nursing standards and procedures
Requirements
- Current Registered Nurse (RN) license in the state of Texas and current CPR certification.
- Minimum of 1 year of nursing experience
- Experience in home health or hospice care preferred
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong critical thinking and problem-solving skills
- Valid driver's license and reliable transportation
