What are the responsibilities and job description for the Case Manager - Safe Haven 3 position at FrontLine Service?
As part of a multidisciplinary team, provides case management services to formally homeless disabled individuals living in Permanent Supportive Housing (PSH) by providing services reflective of an integrated service delivery model, using evidence based best practices such as trauma informed care, motivational interviewing, harm reduction, and IDDT. Develops and implements the goals and objectives in the client’s Individualized Service Plan (ISP) to maintain housing and meet the individual’s identified goals, including but not limited to: decrease in distress caused by mental health symptoms and substance use issues, improvement in physical health, increased income, improved interpersonal relationships, and increased self-sufficiency and self-determination.
This is a non-exempt, union position with a minimum-starting wage of $21.00 per hour.
The hours of work are 8:00am – 4:30pm, Monday through Friday.
Qualified candidates must possess a Bachelor’s degree in social, behavioral or human services, coupled with at least one year of professionally supervised work in the delivery of mental health, chemical dependency or social services. Valid driver’s license with less than six points, proof of insurance along with operational vehicle, required.
